Access our patient forms that you can fill out before your next visit to Nice Eye Care in Burlington.

Our New Patient Exam includes a comprehensive eye health examination including dilated exam of the retina and baseline images and photos of the back of the eye which has a $29 fee in addition to your routine eye exam copay. Some patients will have medical issues which will require that we bill your visit to your medical insurance. If you have any questions about how your exam will be billed, please ask at the time of scheduling.
